This form must be filled out in it's entirety or your application will not be processed.

BY ADOPTING FROM THIS FACILITY YOU ARE AGREEING TO BRING YOUR ADOPTED DOG BACK TO BE SPAYED/NEUTERED ON A DATE WE WILL PROVIDE IF YOUR APPLICATION IS APPROVED.    

I VERIFY THAT I HAVE ANSWERED ALL OF THE QUESTIONS TRUTHFULLY.  I UNDERSTAND IF ANY OF THIS INFORMATION IS FOUND TO FALSE, THIS SHELTER MAY DECLINE THE APPLICANT &/OR CONFISCATE THE ADOPTED DOG.  WE RESERVE THE RIGHT TO REFUSE PLACEMENT OF ANY DOG. APPLICATIONS ARE APPROVED BASED ON QUALIFICATIONS, NOT ORDER THEY ARE RECEIVED.  IN MOST CASES, YOU HAVE 24 HOURS AFTER NOTICE OF APPROVAL TO ADOPT BEFORE ANOTHER ADOPTER WILL BE NOTIFIED OF SHELTER ANIMAL’S AVAILABLITY.

In most cases, the shelter will require a meet and greet with the entire household and current pets. If this is not possible and the dog does not work out, in most cases a refund of the fees would not be approved.
